This form is necessary in situations of divorce, where both parents have 50/50 custody of the minor. In situations like this, both parents are required to consent to treatment. This ensures that mental healthcare providers have the authorization to provide appropriate care for minors, while also protecting their rights and well-being.
The answer to this question is entirely up to you. Therapy is very much dependent on each individual and the goals they are interested in achieving. Some desire short-term goals, which could last anywhere from 6-12 sessions, while others are wanting to accomplish some long-term goals which could last a year or more.
