TP’s MUST be signed by the client.

Treatment Goals

  • Individual Client Goal
  • Ensure the goal is tied to treating the diagnosis given
  • Statement about the client being offered a copy

Objectives

  • A strength-based statement is required.
  • A concrete form of measurement needs to be used:  A likert scale is best 1-10, 30% is difficult to measure so avoid this type of measurement.
  • How progress will be measured
  • Must have at least two objectives
  • For minor client - must have a parent (guardian) objective (so a total of 3 objectives)

Treatment Strategy/Intervention - Modality - Frequency - Estimated Completion

At a minimum, include the treatment strategy/interventions you anticipate using with your client during treatment that are specific to that objective (see TP Expectation document for additional options).

Discharge Criteria/Planning

Discharge Goal

If the client has SI, an objective needs to be focused on it.  This is in addition to the required two objectives - so there would be at least three total.

The TP goals and objectives must align with the diagnosis and be appropriate for treating that diagnosis.
