Termination Letter

A therapist is required to create and send a termination letter to every client once treatment has ended, regardless of the reason, to conclude the therapeutic relationship formally.  This provides clarity to the client, and it helps avoid any implication that the therapist has an ongoing therapeutic responsibility.  

Use this form to produce a termination letter.  This form will enable you to search for matching referrals (see our Referral Network) and input them directly into the letter.  Upon submission, a PDF will be available for download, add to TN and send via the Client Portal.

Do I need to mention the diagnosis or other theraputic treatment?

  • No.  Don’t mention confidential therapeutic treatment information

Do I need to send the termination letter to the client?

  • Yes, via the Client Portal.

Do I still need to fill out this termination letter/form if the treatment was successful?

  • Yes.  

Do I need to complete this termination letter/form if I am going on leave/FMLA leave?

  • Yes.  If the clinician will be taking leave for any reason for more than 1 month, a termination letter needs to be written for each of your clients so that the file reflects the client’s decision about what treatment will look like during the clinician's leave.

Do I need to complete this termination letter/form if resigning from Overcomers?

  • Yes.  Since the clinician will be leaving Overcomers on a set date, a termination letter needs to be written for each of your clients before the clinician leaves, so that the file reflects the client’s decision about what treatment will look like after the last day for the clinician.
