Terminated Client wants to Return to Therapy

If an ex-client (former client, terminated client, etc.) has contacted you and wants to re-engage or return to restart treatment, please answer these questions and then follow the steps below:

  • Has the client been terminated (meaning there’s a termination note on file)?
  • When was the client terminated?

The answer to these questions will provide instructions on what to do below.

If the client was terminated within the last ~3 months

If the termination is recent (within a few months), the client does not have to go through the Scheduling team and book on the directory. Schedule the client as desired.  To become an ‘active client’ again, the client must sign a new Disclosure Statement and Consent for Services, as well as the About Me form.  Instruct the client to update the insurance form in the Client Portal if their insurance details have changed.

If the client was terminated 3+ months ago

If the termination occurred 3+ months ago, it is best to have the client complete the Intake Packet again so we can review/collect any consents (presenting problem, insurance details, etc).  Use the Refer Client form below to book this ex-client and trigger the Intake Packet.

Select the 'new client for myself' option.

The Scheduling team will follow standard procedures and treat them like any other Intake booking.  However, if the client completes the Intake Packet, they’ll be scheduled in TN as a session, not an Intake, because we can only bill (1) 90791 within a year.  After the scheduled appointment, the client MUST sign a new Disclosure Statement and Consent for Services and About Me — the typical process for becoming an active client again.  

If there’s no termination note

No additional paperwork is needed because, technically, they are still your client. You can schedule them on your calendar just as you would any other active client.  Book a session, not an Intake, because you can only bill (1) 90791 within a year.
