Sample Concrete Forms of Measurements

Likert Scale

  1. …implement 2-3 coping skills such as progressive muscle relaxation and four square breathing to decrease their anxiety from a 7 to a 3 on a 1-10 scale with 1 being no anxiety.
  2. The client will rate her overall self-worth with the goal of achieving 7/10 with 10 feeling she had achieved self-worth that week.
  3. …decrease symptoms of anorexia such as fear of specific foods from a 9 to a 4 on a 1-10 scale with 1 being no fear.

Use of Skill Frequency (daily, each week, per month, etc.)

  1. ….in order to create 2-4 effective problem-solving strategies to be used at a minimum of one time daily to be evidenced by increased ability to cope with the stressors of life, increased organization, decreased feelings of anxiousness and overwhelm, and increased overall happiness.
  2. The client's goal is to self-report major depressive disorder symptoms no higher than 2/10 each week while embracing 2-3 thought-challenging skills in her daily life.
  3. …..by using "I statements" 4 out of 7 days a week to work on being more assertive and developing more trust in their relationship.
  4. Progress will be measured by the client writing down at least three positive interactions he has had with someone each week.

SUD Score

  1. …process 3-5 major traumatic memories to reduce her SUD from distress from triggers from a 7 to a 5 (0 = no distress, 10 = worst distress)
  2. The client will decrease her anxiety symptoms from a current SUD of 8/10 with 10 being the worse to a 5/10 or lower.
  3. …engage in EMDR to decrease the subjective unit of distress (SUD) from anything higher than a 0 to a 0 or 1 on a 0-10 scale where 10 is the highest level of distress.

Assessment Scores

  1. Progress will be measured by a minimum of a 5-point reduction in the client's self-report symptoms on the GAD-7.
  2. Progress will be measured by client's self-report of a 10-point reduction in scores on the PHQ-9.

Only time 100% or 0 can be used.
