Resign Disclosure Statement & Consent for Services Every January

To maintain compliance, it's recommended that every ACTIVE client resigns the Disclosure Statement & Consent for Services and About Me.   The Disclosure Statement outlines the client-clinician relationship, so there may have been changes since the last time the client signed it, even if it was just a month ago.  Also, the About Me may have had changes as well, such as supervisor changes, contact info, licenses, etc. and therefore sharing this with the client and having them resign will keep them up-to-date with your information.  

Clinicians must re-send the Disclosure Statement & Consent for Services & About Me to each of their active clients every January.  If an Intake started treatment in January, clinicians do not need to re-send these documents.

How to Resend Disclosure Statement & Consent for Services and About Me's

Go to each client's file portal tab and share the two documents.  During your upcoming sessions, remind/have the client sign them.   Learn more here:  Send and Process Documents/Assessments.

If your About Me has incorrect information on it, please email the Support Team and have them correct it.
