When an existing minor client (under 17 years old) has now become 18, please follow these steps:
- Replace the email address on file with the minor's email. This is to ensure only they have access to the Client Portal. This step must be completed first so that the appropriate person can sign the following consents.
- You may also update the phone number (if not already) to the minor's phone number so that appointment reminders are sent to the correct location.
- Resend the Disclosure Statement, since this document was most likely signed by the parent/guardian.
- Resend the 'About Me' document, as it was most likely signed by the parent/guardian.
- Send the HIPAA Notice of Privacy via the Client Portal (document is already in the TN Library).
- Send the Telehealth Consent via the Client Portal (document is already in the TN Library).
- If the client is Medicaid, resend the Medicaid Client Rights and Responsibilities (Health First Colorado) document, as it was most likely signed by the parent/guardian.
- If the client is Medicaid, resend the Medicaid Members Notice (Health First Colorado) document, as it was most likely signed by the parent/guardian.
- Update the contacts on the client's file (e.g., MOC and/or FOC) to remove appointment reminders and remove the ROI signed date. Removing the ROI Signed Date field indicates that the contact doesn't have authorization from the client to access records or discuss treatment. You can leave the contact information listed in the client's file just for reference.
