Step 1:  Go to:

Step 2: Click on Continuing Competency and Resources

Step 3: Click on Continuing Professional Competency

Step 4: Go to the Resources section

  1. Complete the Professional Practice Survey and Self-Assessment
  2. Complete the Learning Plan

The Professional Practice Survey and Self-Assessment along with the Learning Plan do not need to be sent to DORA, but instead must be kept by the therapist for 5 years as DORA can request these documents for up to that period of time.  (This includes any CEs the therapist has acquired in their renewal process.)
