Employee Complaint/Concern Investigation

Thank you for coming forward and expressing your complaint/concern. Rest assured that a fair, impartial investigation with appropriate next steps will be taken once the form has been reviewed.

To launch an investigation or report a concern about a staff member, follow these steps:

  1. Login into Paylocity
  2. Select Performance from the Menu
  3. Select Create Journal
  4. Select the Employee Complaint/Concern template.
  5. In the left-hand menu, be sure to select who this complaint/concern is about:
    1. "Who's this journal about?"  = Someone Else
    2. Click "Add About Person" to choose a staff member  
  6. Fill in the required information about your complaint/concern of said individual.

Your complaint/concern will be directed to Garrett Luttman and Jennifer Luttman for review.  Your name will remain anonymous as we investigate the incident.   Thank you for raising your concerns with the admins.
