Parents are Married
- 1 parent's consent is required. The Consent to Treat Minor form is located inside the Intake Packet.
Parents are separated but legally married
- 1 parent's consent required, 2nd parent consent contact attempt required. The Consent to Treat Minor form is located inside the Intake Packet.
- Consent for Child of Separated/Divorced Parents (located inside Intake Packet)
- The CLINICIAN should make a Contact Note in TN expressing the contact attempt with the ‘other’ parent (or add a printed PDF of the email to the other parent)
Single Parent - the other parent is MIA
- 1 parent's consent is required. The Consent to Treat Minor form is located inside the Intake Packet.
- Custody Agreement (required to be uploaded in the Intake Packet). CLINICIANS will need to verify that the proper agreement has been received.
Sole (100%) Custody after divorce
- 1 parent's consent is required. The Consent to Treat Minor form is located inside the Intake Packet.
- Consent for Child of Separated/Divorced Parents (located inside Intake Packet - if divorced)
- Custody Agreement (required to be uploaded in the Intake Packet). CLINICIANS will need to verify that the proper agreement has been received.
51+ Custody by Parent
- 1 parent's consent is required. The Consent to Treat Minor form is located inside the Intake Packet.
- Consent for Child of Separated/Divorced Parents (located inside Intake Packet)
- Custody Agreement (required to be uploaded in the Intake Packet). CLINICIANS will need to verify that the proper agreement has been received.
50/50 Custody
- 2 parents' consent required
- Consent to Treat Minor form (located inside the Intake Packet).
- Consent for Child of Separated/Divorced Parents (located inside Intake Packet)
- Consent to Treat Minor by the other parent (the parent not filling out the Intake Packet). The Scheduling Team sends this other consent form after the Intake Packet is received.
- Custody Agreement (required to be uploaded in the Intake Packet). CLINICIANS will need to verify proper agreement is received.
50/50 Custody with authority
- 1 parent's consent is required. The Consent to Treat Minor form is located inside the Intake Packet. If authority is given to 1 parent concerning behavioral health decisions, that parent's consent is required.
- Custody Agreement (required to be uploaded in the Intake Packet). CLINICIANS will need to verify that the proper agreement has been received.

The Intake Packet contains most of the necessary consent forms. However, please verify if any documents are missing.

Which parent signs the Disclosure Statement and About Me?
Only one parent needs to sign the Consent for Services and About Me. This parent will be the one whose email is attached to the portal. The other parent has already consented to treatment in the ‘Consent to Treat Minor Packet’.
