Consultation Notes

Consultation Notes are clinical notes that document our communication and collaboration with other providers (Supervisor, PCP, Psychiatrist, Psychologist, School Counselor, etc.) when we need to consult about challenging cases, medication management, or care coordination. These notes prove that a consultation took place.

Create Consultation Appointment

A Consultation Note requires you to add a “Consultation” appointment to your TN calendar for the day and time the consultation occurred.  

In TN, change the appointment Type to Consultation (see below), select the client for whom the consultation is scheduled, and ensure the date and time are accurate.  You can adjust the meeting duration to whatever is appropriate.  Save the new appointment.

Then, after the consultation occurs, write the consultation note.  Here is an example:  

Service Codes for Consultations

As per our Disclosure Statement:
We may occasionally need to discuss matters outside of regular sessions or consultations with third parties may be required. Calls or communications under 10 minutes are free.  If they exceed 10 minutes, a $20 self-pay fee applies, with an additional $20 for every 10-minute increment thereafter. These fees cannot be billed to insurance.

Service Code:  FREECONSULT

Choose the 'FREECONSULT' service code on the appointment window when the consultation lasts 10 minutes or less.  This type of consultation is free to the client and clinician.

Service Code: 00000 Consultation (1 unit / 10 mins)

Choose the '00000' service code on the appointment window when the consultation lasts 10 minutes or longer.  This type of consultation creates a bill for the client of $20 for every 10 minutes entered on the appointment.  For example, if the consultation lasts 35 minutes, the charge to the client will equal $80.  

0-10 minutes = $20
10-20 minutes = $20
20-30 minutes = $20
30-35 minutes = $20

Ensure the client is informed of these charges BEFORE the consultation with the third party.

Service Code: T1017 - Medicaid Clients

Medicaid members cannot self-pay for consults because Medicaid offers a T1017 service code:  T1017 Consultation Note.  If your client is a Medicaid member, please use the T1017 service code.
