Why do I have to complete this entire Intake Packet?
There are two primary reasons. First, insurance providers and/or DORA (Department of Regulatory Agencies) require specific questions and consents to be collected when an individual seeks counseling services. Second, most of this information is necessary to better understand why you are seeking counseling and to collect payment information.
Where do I provide you with my insurance information?
You can provide us with your insurance information on page 5.
Will I lose this booking if I don’t complete this packet?
Yes. Without this information, we wouldn’t have the proper consents in place.
Can I fill this packet out in person?
Unfortunately no. Our Intake Packet and most other forms can be easily completed from your computer or phone.
